טופס הצהרת ויתור על אזרחות ישראלית בעבור קטין להורדה

טופס זה מיועד להצהרת ויתור על אזרחות ישראלית הניתנת ע"י הורה/ים בעבור קטין כאשר ההורה/ים והקטין הם תושבי חו"ל, והאזרחות נקנתה לקטין/ה מכוח לידה בחו"ל להורה אזרח ישראל. הטופס ייחתם ע"י שני ההורים. בהיעדר הסכמה אישית של ההורה השני, יש להציג פסק דין או הסכם גירושין מקורי ומאושר על ידי בית המשפט המציין כי האפוטרופסות הבלעדית על הקטין ניתנה להורה מגיש ההצהרה, או פסק דין הקובע כי ההורה המצהיר רשאי להצהיר עבור הקטין לבדו. אם מלאו לקטין 16 שנים, עליו להודיע בכתב שהוא מסכים לביטול אזרחותו. אני/ו הוריהו/יו של הקטין/ה הר"מ מצהיר/ים בזה כי ילדי/תי הר"מ הינו תושב חו"ל ואני/ו מוותר/ים על אזרחותו הישראלית.

 This form is intended for a declaration of a waiver of Israeli citizenship submitted by parents on behalf of a minor, the parents and minor being overseas residents and the child acquiring citizenship by being born abroad to a parent who has Israeli citizenship. The form is to be signed by both parents. In the absence of personal consent of the other parent, a court ruling or an original divorce agreement approved by the court, stating that custody of the minor was granted to the parent submitting the application, is to be produced; or a court ruling stating that the applying parent may submit a declaration on behalf of the minor by him/herself . If the minor is 16 years of age or older, he/she must notify in writing that he/she agrees to the waiving of his/her Israeli citizenship. I/we, the parent(s) of the minor below, declare hereby that my/our child below is a foreign resident, and I/we waive his/her Israeli citizenship.
